Respirator Medical Questionnaires

To ensure that wearing respiratory protection doesn’t cause undue health concerns, OSHA requires you to “provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace.”

After the questionnaire has been reviewed by the Physician we will provide you with a written document summarizing the physician’s opinion.
